Geriatric Care Management
The care manager's assessment and plan of care address all aspects of the client's life and
include the coordination of the myriad of services essential to meet their daily physical, mental and emotional needs.
Services may include
medication management, assurance that medical
appointments are kept, home safety, nutritional guidance, and
recognition of depression or dementia.
The care manager supervises the administration of the care plan, advocates for the client, and provides information on community resources and placement options. They work closely with family and significant others to promote the best interest of the client. Care managers provide the important communication link between the client and long-distant family members, physicians, and legal and financial professionals.
