Initial Consultation
A complimentary initial consultation
personally introduces you to Life Care Solutions. A professional
care manager meets with the prospective client and/or family or
responsible party to identify individual needs and concerns. The
care manager will summarize support and services to be provided,
make recommendations, and outline the costs.

Evaluation
A comprehensive evaluation includes
the following areas:

activities of daily living |

medication management |
 |
nutrition |
 |
psychosocial adjustment |
 |
mobility |

house safety and cognition |
The report generated summarizes the
results of the evaluation, concerns, and recommendations.

Care Management
All services are based on the
individual needs of the client and may include:

Completing a thorough evaluation
and presorting a comprehensive plan of care to address all
aspects of care for an individual over time. |

Serving as a single point of
contact to coordinate care among providers and acting as a
liaison with family members. |
 |
Coordinating and accompanying
clients to medical appointments assuring the best possible
follow-through. |
 |
Providing clear answers and
directions to navigate the healthcare system and advocate for
the highest quality of care. |

Intervening in and managing a crisis
situation. |
 |
Providing services 24 hours a day, 7 days a week. |

Care Manager Assurance
This program is specifically designed
for individuals and families who would like to be connected to all
the resources care management can provide, including support and
occasional "checking in," but don't require frequent services.
Program Features:

An initial assessment by a
professional care manager in your current home. |
 |
Partner with you to design a
personal plan to include current support and future preferences. |
 |
Quarterly contact with the care
manager to review changes, identify additional resources, and
update your personal plan. |
 |
Should an emergency arise or circumstances change and you or
your family member needs more assistance, a care manager is on
call 24 hours a day, 7 days a week. |

Crisis Intervention
Many challenges can become a crisis in
one's life: discharge from a care facility, adult protective
services or
senior victims intervention, relocation, significant change in
functional status, loss of spouse or primary caregiver.
Care management services are available
to help you through these difficult times. Service may be temporary
or ongoing.
