How we work with you:

  • We start with your challenges to determine how we can best serve you.

  • We will develop a plan with clear objectives governing how we will help you and your family.  It may be an assessment, or management of care providers, or dealing with family dynamics.  We will tailor the plan for your specific needs.

  • We will assign a Care Manager based on the needs of the plan and skills required of the care manager.

  • We will communicate with you on a schedule that suits you to ensure you are aware of everything going on, without being burdened by questions or routine issues.

 How we bill for our services:

  • We work with you as a team to tailor our solutions to fit your needs and budget.

  • Our services are billed hourly in 6 minute increments, plus expenses such as faxes and copies. 

  • Depending on your needs, we may be able to offer a monthly flat rate plan which covers your scope of services.

  • In most cases, our services are not covered by insurance, however some long-term care insurance policies and workman’s comp insurance do provide coverage.  We recommend checking your long-term care policy for coverage of our services.  The following providers offer coverage of Care Management Services, also referred to as Case Management Services:
