Functional Assessment

A Functional Assessment is an evaluation of the physical and cognitive ability of the client and their living situation and environment.  A Care Manager will conduct an in depth assessment at the client's home or living community apartment.  This 8 to 10 page Assessment covers background, medical, social, legal and financial findings and includes diagnostic tests for balance, movement, cognition, and depression.  The assessment provides clinically recognized indicators for medical follow up and is a valuable tool in determining an appropriate living environment.
